Behind the deductible
The four copays above cover everyday care. Everything else on this plan runs through your $7,350 deductible, and several services need pre-certification first.
- Hospital & facility care Inpatient, outpatient surgery, anesthesia. Pre-certification required. Confirm the inpatient facility share with your agent.
- Labs, radiology, CT, MRI and PET Through the deductible. CT, MRI, MRA and PET need pre-certification.
- Emergency room & ambulance No copay. Both run through your deductible.
- Prescriptions Preventive generics are $0. Every other prescription runs through your deductible.
