PHCS PPO network

This plan runs on the PHCS PPO network. The team can check your doctor and your hospital by name before you enroll.

Pre-certification is required on many services, including hospital stays, surgery, imaging, tests and labs, mental health care, home health, rehabilitation, skilled nursing, medical equipment, hospice and preventive care. Without it, the service is not covered.

High-cost services

The big stuff, after the deductible.

You pay 40% after your $6,000 deductible,
then the plan pays 100% once you reach the $12,000 max out of pocket.

Hospitalization Inpatient hospital stays. Pre-certification required, or it is not covered.
Outpatient surgery Same-day procedures, at the facility and the surgeon.
Diagnostic labs 40% after the deductible; basic labs a $50 copay.
Imaging CT, PET and MRI scans. Pre-certification required, or it is not covered.

In network, and subject to plan terms, limitations and exclusions. Pre-certification is required on many services, including hospital stays, surgery, imaging, tests and labs, mental health care, home health, rehabilitation, skilled nursing, medical equipment, hospice and preventive care; without it, the service is not covered. Premiums, balance billing, penalties and care the plan does not cover do not count toward the out-⁠of-⁠pocket max.
